Guide to Documentation

This guide is meant to assist new therapists in formulating clinical notes. After reflecting with several colleagues of varying experience levels, I realized that many (if not most) of us were actually taught how to write our notes. I hope this will help emerging clinicians to feel more confident in their documentation abilities!
