Psychotherapy Treatment Plan Template

Are you looking for a visually-appealing way to take notes for your treatment plans? Do you find that you sometimes don’t write down certain details, only to need them later? I created this template to make this often-avoided documentation easier and more pleasant. It works well with any EHR, but I based it off of several templates in Simple Practice.  There is space for general information about the treatment plan, as well as up to three diagnoses, three goals, and up to three objectives and three interventions for each goal.  

Like most of my resources this one is designed to be visually appealing by using simple graphics and creative fonts (sometimes people find that they feel more motivated to keep working on something when they find it aesthetically pleasing!). Yet the design is simple enough to not be “busy” or interfere with the purpose of the template.

This resource is both printable and available as a fillable PDF, perfect for telehealth. In addition to being able to have a print copy, you can also easily save it to your health records system.
