When ending with clients, I have often wanted them to have something to serve as a reminder of their progress and how far they have come, as well as internal and external resources to use when they struggle in the future. I made this 2-page handout to serve this purpose as well as to help structure the final session. In addition to some reflection questions to complete together, there is space toward the end for the therapist to write a quick note/letter to the client. After the final session, this handout should be sent to the client for them to look back on when they need a reminder of the skills, progress, and connection they gained in therapy.
This handout is printable and fillable, so it can be completed either in-person or via telehealth and then handed to or emailed to the client, as well as uploaded to client charts.
This is meant to be a collaborative worksheet, and therefore works best when the decision to end therapy is a mutual one. It is not meant to replace discharge summaries or other documentation, but is intended to be something helpful and meaningful for the client to take home and something to help you structure your last session. Because of this, I find that I do not use this handout with all clients, but it is helpful when appropriate. The population I work with is mostly teen girls and young women, so the font and overall design reflect that, though the questions/prompts are applicable to any population.
